Payment Information
ORMS uses a service to provide online payment (Visa or MasterCard only). This credit card service is designed to provide e-commerce for which shipping information is important. We do not need to ship anything to you, but for the card transaction to be complete we request that you complete the shipping information section on the following page. You do not have to complete the billing information section unless that information is different from the shipping information; then you must complete both sections.
Also, you must select a product. O1 is registration before 5 PM PT, February 1, 2010, and carries a charge of $30 for up to five program selections. There is a $10 fee per unit for each additional program over five selections on your application, and, therefore, you must enter one unit in the quantity block of O2 for each additional program selected.
All additional site requests submitted after your application will be processed at $10 per program, regardless of the number of sites you originally listed. O2 is the mechanism to pay for additional site requests. These requests must be submitted by e-mail; do not submit additional applications.

O3 is for registration after 5 PM PT, February 1, 2010, and before 5 PM PT, March 1, 2010, and has an additional charge of $15.
The vast majority of candidates should select a quantity of one (1) O1, for a total cost of $30, if registering on or before February 1, 2010. The total fee charged by ORMS is reflected in O1 and O2, or O2 and O3, above. Please do not pay multiple times, even if there is an error reported at the end of the transaction. If we do not receive payment, we will notify you of that fact at the time that we acknowledge receipt of your application. Multiple payments will be refunded, but it may take more than a month to rectify your account.
You may pay at any time (at application or until the deadlines listed above), but your application materials on file with ORMS will be held until ORMS receives your payment. At the time that ORMS receives payment, copies of your application will be forwarded to the programs that you have indicated on your application. We have designed the page to make online payment a seamless process that you can complete at registration to lessen paper work and decrease the chance for error.
